2. Start Your Application — Three Ways to Apply

You can apply for benefits in any of these three ways:

Online (if you’re comfortable using the internet):

Go to www.ssa.gov/applyfordisability.
 You’ll fill out a disability application and a work history form. You can save and come back later if needed.

By Phone:

Call 1-800-772-1213 (TTY: 1-800-325-0778).
 A Social Security representative will take your information and set up a phone appointment if needed.

In Person:

You can call ahead and schedule a meeting at your local Social Security office. In-person help is great if you don’t have internet access or if the forms seem overwhelming.

3. What You’ll Need to Provide

Before you apply, gather this information:

  • Your full name, Social Security number, and birth date.
  • Names and addresses of all doctors, clinics, and hospitals you’ve been to.
  • A list of medications you take.
  • Your complete work history for the last 15 years (including job titles and duties).
  • Your education and any job training or certifications.
  • Marriage and family information (if you have children or a spouse, it could affect your benefits).
  • Your bank account number (for direct deposit if approved).

You don’t need to have every single detail perfect — but the more complete your information, the better your chances.

4. After You Apply

Social Security will send your claim to your state’s Disability Determination Services office. They may:

  • Request medical records from your doctors.
  • Send you to a consultative exam with a doctor they choose.
  • Ask you more questions by mail or phone.
